SPIDER VEINS - LEGS AND FACE

Did you feel like you did not want to wear shorts last summer? Do your cheek or nose veins cause your face to look dark red or purple in cold weather or with exercise? If so, treatment is just around the corner.
Dr. Swinehart has treated these superfluous, unnecessary leg veins for 20 years with sclerotherapy. The hypertonic saline used for this purpose has the advantage of not causing allergies. You have saline at a concentration of 0.9% in your blood stream; we use a concentration of 23.4% for sclerotherapy. Your veins are simply tubes of living cells. The saline eradicates these cells, causing the unwanted veins to disappear.

Although more than one treatment may be necessary, sclerotherapy has an advantage over laser therapy in both cost and efficiency. Using a laser, the doctor has to deliver a pulse every few millimeters along each and every vein. With sclerotherapy, however, one injection into the trunk of the "tree" may cause all of the "branches" to disappear!

Smaller numbers of facial veins may be treated in the office with the electric needle (cautery); the pulsed dye laser is used for larger areas of numbers of "spider veins" on the nose, cheeks, and face.

EXCESSIVE SWEATING
For people with this problem, hyperhidrosis (excessive sweating) is no laughing matter.

One of Dr. Swinehart's patients, a college professor who wore a shirt and tie, was aware that his students were betting on whether or not his armpit sweat stain could reach down to his belt line before the bell would ring. Other patients are unable to accept jobs that involve shaking hands.

If Drysol® fails, sweat glands in the axillary (armpit) area may be permanently removed under local anesthesia through three tiny (1/8 inch) incisions with a suction cannula in the office. This tumescent liposuction technique often results in an 80% ro 90% reduction in armpit sweating.

BOTOX®, all the rage for treatment of wrinkles, works for sweating in both the hands and the feet, as well as the armpits. The effects of Botox® generally last approximately four to six months.

TREATMENT OF SUN DAMAGE AND WRINKLES

     

So many treatments for wrinkles, unevenly pigmented skin, and sun damaged skin have been developed over the past decade that no one has to accept this any more. MICRODERMABRASION, utilizing a stream of aluminum oxide crystals, is performed every 1 to 4 weeks, is inexpensive, and has the advantage of no time lost from work or activities.

     The new COLORADO PEEL is showing great promise for therapy of sun damaged skin, photoaging, irregular pigmentation, and fine facial lines. These series of treatments combine microdermabrasion with a light chemical peel and / or dry ice treatment, chosen by Dr. Swinehart to match your particular condition. The advantage? Little or no "down time," along with a reasonable price, for each procedure.

     For more advanced sun damage, DERMASANDING has produced some remarkable results, as seen in the photos.

     The skin can also be smoothed out with radio frequency current, cryotherapy (liquid nitrogen or carbon dioxide), or DERMABRASION. In addition, lasers frequently are used to remove wrinkles effectively - either the "cool" lasers (creating increased dermal synthesis of collagen) or ultrapulsed carbon dioxide laser may be utilized. Dr. Swinehart is happy with the laser peel that he had in 1999.

     CHEMICAL PEELS, used for decades, are very effective for the treatment of facial wrinkles. Chemicals employed include Trichloroacetic Acid, Jessner's, Glycolic Acid, and Phenol. The "COOK PEEL," developed in San Diego, is often used for dark spots on the hands, forearms, neck and rest of the body.

     Deeper lines must be filled in via SOFT TISSUE AUGMENTATION. This takes place at one of four levels:

     COLLAGEN (Zyderm®, Zyplast®) is placed in the upper dermis, right under the epidermis, for the finest lines on the face, around the lips, on the cheeks, and for "crow's feet."

     SUBCISION / DERMAL GRAFTING augments the mid-level tissues with your own collagen, often taken from the crease behind your ear.

     FAT INJECTION, used for deeper and larger skin folds, uses your own fat and does not carry the risk of an allergic reaction.

     BOTOX® stops the muscles of the expression from contracting and can be used for the frown lines between the eyes, forehead lines, "crow's feet" between the eyes, and prominent neck bands.

SAGGING NECK SKIN OR DOUBLE CHIN

Unwanted or excess fat in the jowls, lower face, neck, and "double chin" can be nearly completely removed via TUMESCENT LIPOSUCTION, performed under local anesthesia in the office (see www.coloderm.com for more photos).

The hundreds of tiny tunnels thus created then tighten up as they heal, drawing the skin back upward instead of sagging. Similarly, if one filled a sponge with glue, put a book on it, and came back in the morning, the book should be glued down permanently. The EXCESS SKIN can be tightened up additionally with a few dissolving stitches under the skin through a small incision under the chin, without the large incisions, recovery time, and high cost of a face lift.

PROMINENT NECK BANDS (platysmal muscle) can be successfully injected every few months with BOTOX®.

SUN DAMAGED SKIN on the neck can be treated with pigment lasers, chemical peels, microdermabrasion and Cook peels.

EYELID BAGS, LOOSE SKIN, AND WRINKLES

Do people tell you that you look tired in the morning, even though you feel rested? Would you again like a place to put your eye shadow?

LOWER LID BAGS are simply fat pads - not from allergies, lack of sleep, water gain, or rubbing. With TRANSCONJUNCTIVAL BLEPHAROPLASTY, these bags are removed through a small incision in the pink part (inside) of the eye, leaving no scar. This procedure is performed in the office under local anesthesia.

DARK CIRCLES under the eyes can be treated with lasers, chemical peels, retinoic acid (Retin A®), and/or Hydroquinone (Triluma®).

EXCESS SKIN on the upper lids is removed with a BLEPHAROPLASTY, performed in the office. A tiny scar will remain hidden in the new eyelid crease. Dr. Swinehart himself has had both upper and lower lids succesfully treated this way in 1993.

WRINKLES on the lower lids are not cut out, but are best treated with chemical peels, dermasanding, or dermabrasion.

"CROW'S FEET" (smile lines outside of each eye) respond quite well to BOTOX®, as well as to dermasanding and dermabrasion.
